  • Conduct a comprehensive geriatric and home safety assessment<\/li>\n\n\n\n
  • Develop a geriatric plan of care<\/li>\n\n\n\n
  • Visit the home or care setting routinely<\/li>\n\n\n\n
  • Establish a personal relationship with the senior<\/li>\n\n\n\n
  • Provide round-the-clock support wherever we’re needed<\/li>\n\n\n\n
  • Assist with insurance and Medicare issues<\/li>\n\n\n\n
  • Manage medications<\/li>\n\n\n\n
  • Provide outside referrals for at-home skilled nursing care\u00a0<\/li>\n\n\n\n
  • Train and supervise in-home staff<\/li>\n\n\n\n
  • Supervise hospital and nursing home care<\/li>\n\n\n\n
  • Coordinate medical services and care<\/li>\n\n\n\n
  • Accompany the senior to physician visits or outpatient procedures<\/li>\n\n\n\n
  • Serve as liaison between the client and medical, social service, and\u00a0psychiatric professionals<\/li>\n\n\n\n
  • Provide referrals for professional therapeutic counseling services<\/li>\n\n\n\n
  • Facilitate hospital or nursing home discharge<\/li>\n<\/ul>\n\n\n\n
